Supporting Successful Transitions

Transitions are a natural part of life, and at Sacred Heart, we recognise that starting and finishing primary school are two of the most significant transitions in a child’s educational journey.

Foundation Transition – Sacred Heart Little Learners Program

To support our incoming Foundation students, we offer the Sacred Heart Little Learners program, which runs weekly throughout Term 4. This program invites enrolled students for the following year to attend one morning each week to:

  • Become familiar with our school environment

  • Get to know our staff, routines, and expectations

  • Begin building relationships with peers and members of our school community

This early engagement also allows our staff to begin understanding each child’s individual needs in preparation for a smooth start to school life. The Little Learners program is a joyful and much-anticipated time for our whole community, as we warmly welcome the next generation of Sacred Heart learners.

Parents will be provided with detailed information about the program, including session times and dates, well in advance.

Year Six Transition to Secondary School

For our Year Six students, transition to secondary school is also carefully supported. Students are given opportunities to:

  • Visit their future secondary schools

  • Meet and speak with current secondary students who share insights about what to expect

Where additional support is needed—especially for students with specific learning or social/emotional needs—we work closely with families to plan for a successful transition. These conversations are held individually to ensure every student receives the care and consideration they deserve during this important time.
